As the others have said - it is real, but it's not something you can just do.

It's something you work up to slowly over a long time, stretching your ass and training your body.

Going form zero to a fist would be impossible, and would probably kill you. It's pretty extreme stuff, but apparently, if you train properly, it is safe. It's something you need to be VERY careful with though.
