Ha ha,
Happened to me last week.
I got out of the car in a parking lot and in the space next to me a woman is talking on her cell phone in the drivers seat. I start staring at her, cause she looks just like my niece who lives in the area. So I'm staring at her and I look around the inside of the car and realize, hey, there's no baby seat, the cars to clean no baby toys around, look back and the woman and realize, OH, that's not who I thought it was, so I casually turned away and left. I bet I freaked that woman out big time. Bet she was glad to have a cell phone the way I was checking her and her car out.
Happened to me last week.

I've been in that embarrassing situation a few times. I have more often had the other one - the one where I don't speak to someone because I am not really sure it's who I think it is, and they think I am ignoring them.

About three years ago I had a weird conversation with some guy who said "Didn't you train at....?" (he named a certain gym in town).
Indeed I had., VERY briefly. I did one, possibly two at most, casual workouts there before I decided that the other gym across the road had the better deal and joined up there. This was in - wait for it - 1973.
It definitely was me that he remembered, he confirmed the date. The fact that he remembered and recognised me after all that time gave me cause to reflect: wouldn't it be great if I could make a lasting impression like that on all the guys I am trying to seduce? Bizarre.
