Apparently, what happened was that his pants got ripped. Here's what he said during the MTV interview:

C'mon! I know the feeling of being desparately looking for some nude pics of one's favorite actor but this is a fake nevertheless. Sorry to disappoint you but if the pic was real, this would've caused a wide stir

I thought so too. I myself do photomanipulations (but not that kind). I must say it's good. Look so real, including the tone color of his head and body skin.

This Naked Josh Hutcherson Photo Is NOT Real
A year ago or so, 20-year old Josh Hutcherson was allegedly caught on a online dating site.. under the name, “Connor”.‘Connor’ was not only sending photos to girl(s) but actual videos of him pleasuring himself to someone who we gladly got a hold of. The photos, which clearly depict his tattoo, were taken sometime around 2010-2011.
