What would be very helpful would be an additional button for each entry on Forum index pages (perhaps next to the last post button) that took you to the first unread post in a thread. Possible?

It's already a default on vbulletin.
Look for the arrow pointing down next to each thread:

View First Unread ....this is at the top left of every thread, click it and you will be directed to the first unread post in that thread
