If you had your piercing for YEARS and it has not healed, that is a problem. Is the metal surgical steel or gold? If not, take it our immediately!
One of my piercings took awhile to heal too...I used a liquid soap in the shower (NOT antibacterial soap!), something gentle like Ivory or Dove for sensitive skin (i.e. without fragrance). Put the soap all over the metal and gently slide the metal through your nipple to cover all parts of the piercing and get the soap into the inner portion of your nipple - it may sting a little. Gently move the metal only a couple of times for the first few days, after it stops stinging, you can move it back and forth a little more. Make sure you rise off the soap well.
I think I did this for about a month, may be a little longer...just to be sure it was completely healed. The inner portion of your nipple takes a while to heal so make you STOP playing with it during sex (solo or with a partner =( ), do not wear really tight shirts that might rub on your nipple and if you exercise, put a band-aid over your nipple so your nipple doesn't get irritated - or it will never heal!
